Keys to Success accepts most insurance providers. When calling for an appointment, make sure you have your medical insurance card available to communicate with office staff along with the insurance number and co-payments, as well as any other necessary information. We also take private pay based on a general sliding fee rate.
To obtain your driver's license after losing it to the Department of Transportation and/or courts, one must complete the following two items:
1. Complete an OWI/DUI evaluation AND complete the recommendations of the therapist. Cost for evaluation is $125.
2. Attend and complete the 12 hour Prime for Life class that is a DOT State of Iowa court mandated class for driving under the influence. All classes are 2 day sessions beginning Friday evening and ending on Saturday evening. Cost for the class is $180.
A Keys to Success therapist will complete all required documentation upon completion and send documents to those you have listed on your release of information, typically DOT, lawyer and/or court house.
